REGARDING YEARLY W-2 FORMS:

By February 2, 2026, W-2 forms will be available for pick up in hardcopy form at the restaurant. One month following the required date, aNY W-2 forms that have not been picked up, will be mailed to the employees last address on file. Employees can request their W-2 FORM to be provided electronically by sending an email to heather@eatnest.com, stating the request for electronic sending of their W-2 FORM; the past-employee must provide their full legal name and the last 4 digits of their social security number in the request. Requests will be completed and responded to within 48hrs.

COMPANY SEPARATION NOTICES

Required Notices:

  1. EDD For Your Benefit Brochure (DE 2320) – The California Employment Development Department (EDD) requires employers to provide their unemployment benefits pamphlet, For Your Benefit (DE 2320) to all discharged or laid off employees no later than the effective date of the discharge or layoff. In summary, this pamphlet provides information on UI, DI, PFL, and Job Service benefits available to the employee.

  2. Notice to Employee as to Change in Relationship – California Unemployment Insurance Code 1089 requires employers to give a written Notice to Employee as to Change in Relationship form to all discharged or laid off employees immediately upon termination. https://edd.ca.gov/siteassets/files/about_edd/pdf/noticetoemployeeastochangeinrelationship.pdf

  3. HIPP Notice (DHCS 9061) – The California Department of Health Care Services requires employers with 20 or more employees to provide the Health Insurance Premium Payment (HIPP) notice, DHCS 9061, to terminating employees covered under the program. HIPP is a program in which the California Department of Health will provide health coverage for an individual with a high-cost medical condition.   4. COBRA or CAL-COBRA Notice – Describes general COBRA (or CAL-COBRA) rights and employee obligations. This notice must be provided to each covered employee and each covered spouse of an employee who becomes covered under the plan. This is typically sent by your COBRA Plan Administrator.
